Why This Matters
Outpatient psychiatric follow-up within 7 days of psychiatric hospital discharge is the clinical standard. The same applies to ER visits for psychiatric reasons and crisis stabilization episodes. The transition from acute care to outpatient is a known risk period — symptoms can return, medication regimens may need adjustment, and the psychological work of integration begins. Our practice is structured to provide that follow-up within the standard timeline.
Crisis stabilization follow-up is appropriate after psychiatric inpatient discharge, ER psychiatric visits that did not result in admission, intensive outpatient program (IOP) completion, or any other acute psychiatric care episode. The page does not apply to active crisis — for active suicidal ideation, severe agitation, or other emergent presentations, the right resource is 988, 911, or the ER, not scheduled outpatient care.
Common Concerns We See
Hospital discharge handoff
Inpatient discharge with outpatient follow-up scheduled within 7 days reduces readmission rates and stabilizes recovery. Long waits between discharge and outpatient pickup create known risk.
ER visit follow-up
Psychiatric ER visits that do not result in admission still warrant timely outpatient follow-up. Sometimes patients are told 'follow up with outpatient psychiatry' but cannot find an opening within months — we close that gap.
IOP step-down
Intensive outpatient programs typically discharge to standard outpatient psychiatry. Continuity matters; we coordinate with the IOP team for handoff.
Medication adjustments post-crisis
New medications started during inpatient or ER care often need adjustment in the weeks afterward — dose changes, side effect management, augmentation decisions. Outpatient follow-up handles this.
How This Works
Have your discharge paperwork ready when you call intake — medication list, diagnoses, discharge instructions, recommended follow-up timeline. We schedule within 7 days of discharge when possible (often sooner) and obtain records from the discharging facility to inform the follow-up visit. The first outpatient visit reviews what happened, the current regimen, and the plan going forward.
Clinical Perspective
A common scenario in our practice: a patient discharged from a 3-7 day psychiatric inpatient stay with new diagnoses, new medications, and a referral for outpatient follow-up within 7 days. The discharging facility's social worker calls our office on behalf of the patient. We obtain discharge records, schedule within the week, and provide the first outpatient visit as a transition-of-care evaluation. Subsequent visits follow standard outpatient cadence.

Frequently Asked Questions
How quickly can I follow up after discharge?
Typically within 1-7 days of discharge. We prioritize post-acute follow-up scheduling. Discharging facilities can coordinate the referral on your behalf.
Will you communicate with the hospital that discharged me?
With your written consent, yes. Coordination with the discharging team supports continuity. We typically request discharge paperwork, recent inpatient notes, and the recommended outpatient plan.
What if I was discharged with a regimen I'm not sure about?
Bring your questions to the follow-up visit. We review the inpatient regimen, discuss what made sense for the acute presentation versus what fits the outpatient phase, and adjust as clinically appropriate. Discharge plans sometimes need refinement once the patient is no longer in the acute setting.
What if I went to the ER but wasn't admitted?
ER visit follow-up is similar. Bring the ER discharge paperwork and any prescriptions started during the visit. We continue the medications when appropriate and address whatever brought you to the ER.
Will insurance cover the post-discharge visit?
Yes. Post-acute psychiatric follow-up is standard psychiatric outpatient care — coverage matches your plan's behavioral health benefit. We verify before scheduling.
What if I cannot be seen within 7 days?
If our schedule is unusually full, we tell you directly. Sometimes the discharging facility can refer to an alternative provider for the within-7-day window with planned transition back to us. Bridge prescription continuity is typically maintainable in any case.
