How Telehealth Psychiatry Works | East Texas Psychiatry & Counseling
How telehealth psychiatry actually works in Texas — what to expect, what is required, and what the limits are.
Board-certified PMHNPs. Most patients seen within a week. 90-minute first appointment. Same provider every visit. Statewide telepsychiatry from our Tyler clinic.
Why This Matters
Telehealth psychiatry expanded substantially during the COVID-19 pandemic and has remained the dominant model for many practices including ours. Outcomes data shows telehealth psychiatry produces equivalent clinical results to in-person care for most conditions. Patient satisfaction is generally higher because telehealth removes the time, travel, and access burden of in-person visits. This page explains how it works and what its limits are.
Telehealth psychiatry works for the vast majority of adult psychiatric care. Limits include first-time controlled substance prescriptions in some cases (federal regulations may require an in-person evaluation), severe psychiatric conditions requiring direct examination or hospitalization, and patients without reliable internet access or private space. Most patients fit telehealth well; the few exceptions are typically clear at intake.
Common Concerns We See
Equivalent clinical outcomes
Multiple studies show telehealth psychiatry produces clinical outcomes equivalent to in-person care for most psychiatric conditions. Diagnostic accuracy, treatment response, and patient satisfaction are comparable or better.
HIPAA-compliant technology
Telepsychiatry uses platforms specifically designed for healthcare — encrypted, HIPAA-compliant, with no recording or storage beyond what is required for medical records. Consumer video platforms (Zoom personal, FaceTime) are not appropriate for medical care.
Insurance coverage
Most commercial insurance plans, Medicare, Medicare Advantage, Medicaid, and TRICARE cover telepsychiatry at the same rate as in-person visits under federal and Texas mental health parity rules.
Controlled substance considerations
DEA regulations have evolved during and after the pandemic. As of current guidance, some controlled substance prescriptions may require an initial in-person evaluation. We discuss this at intake when applicable.
How This Works
Telehealth visits happen via secure video on your computer, tablet, or smartphone. We use HIPAA-compliant video platforms. You need a private space, a stable internet connection, and a working camera and microphone. The visit itself is structurally identical to an in-person visit — same length, same clinical content, same prescribing capability for most medications. You receive a visit link and instructions before each appointment.
Clinical Perspective
A common scenario in our practice: a patient who was initially skeptical about telepsychiatry but tried it because of long in-person waitlists. After a few visits, most patients prefer telepsychiatry — the time savings, the ability to do appointments from home or office without driving, and the reduced anxiety of not navigating a waiting room all matter. Clinical outcomes are equivalent. The minority who prefer in-person typically value the office environment specifically; we accommodate both.

Frequently Asked Questions
What do I need for a telehealth visit?
A computer, tablet, or smartphone with a working camera and microphone, a stable internet connection, and a private space for the duration of the visit. Most patients use what they already have.
Is telepsychiatry as good as in-person care?
For most adult psychiatric conditions, yes. Outcomes data shows equivalent clinical results. Some specific situations benefit from in-person — typically severe presentations, certain controlled substance situations, or patient preference.
Will my insurance cover telepsychiatry?
Most commercial plans, Medicare, Medicare Advantage, Medicaid, and TRICARE cover telepsychiatry at the same rate as in-person under parity rules. Specific coverage varies; we verify before scheduling.
Can you prescribe controlled substances by telepsychiatry?
Sometimes, with significant restrictions. Federal DEA regulations are evolving. For established patients with controlled substance prescriptions, telepsychiatry typically continues working. For new patients seeking controlled substance prescriptions, an in-person evaluation may be required.
What if my internet drops during a visit?
We pause, reconnect, and continue. If the connection cannot be reestablished, we reschedule or complete the visit by phone. Connection issues do not generate charges.
Can I do telepsychiatry from outside Texas?
Generally no. Texas medical licensure governs Texas-based care. Patients temporarily outside Texas may sometimes be accommodated; check with our intake team for specific situations.
