Why Superbill Matters
A superbill is an itemized receipt with everything your insurance needs to process a reimbursement claim — date of service, CPT procedure codes, ICD-10 diagnosis codes, our provider NPI, and our practice tax ID. We provide superbills automatically after each visit so you can submit them to your insurance for out-of-network reimbursement.
What You'll Pay
If we are not in-network with your insurance, you pay our published rate at the time of service. After your visit, we email a superbill within 1-2 business days. You submit it to your insurance via their member portal, app, mail, or fax (each plan handles this differently). Your insurance processes the claim against your out-of-network benefits.
Cost transparency before your first visit
Our intake team confirms your expected cost in writing before you book. No surprise charges, no deposit required to schedule, no hidden add-ons. If your situation changes (insurance starts, plan changes, billing arrangement shifts), we discuss options at that point.
How It Works
After each visit, you receive an itemized superbill by email. Most insurance plans accept superbill submissions through their member portal. You upload the PDF, complete a simple claim form (typically pre-populated from your account), and the claim processes in 2-6 weeks. Reimbursement is paid to you directly via check or direct deposit, or applied to your out-of-network deductible.
When This Makes Sense
Your plan has OON benefits but we're not in-network
PPO and POS plans usually have out-of-network benefits — typically 50-70% reimbursement after you meet your OON deductible.
You want to use HSA or FSA funds
Even when superbill reimbursement is uncertain, the visit fee is fully HSA- and FSA-eligible at the time of service.
You're tracking out-of-network spending toward a deductible
Superbills document your spending so it counts toward your OON deductible — increasing future reimbursement rates.
You want to keep your options open
Submitting a superbill costs you nothing extra. If your plan reimburses, great. If not, you've still received care at our self-pay rate.
Clinical Perspective
A common scenario in our practice: a patient whose Cigna PPO plan is in-network with us at the medical level but routes behavioral health through Evernorth — where we are not contracted. They see us as out-of-network, pay our rate at the time of service, and submit superbills to Cigna. After meeting their $750 OON behavioral health deductible, they recover 60-70% per visit. Total annual cost typically beats driving an hour each way to an in-network provider.

Frequently Asked Questions
What is a superbill?
A superbill is an itemized receipt with everything your insurance needs to process an out-of-network reimbursement claim: date of service, CPT procedure codes (visit type), ICD-10 diagnosis codes, provider NPI, and our practice tax ID.
How do I submit a superbill to my insurance?
Most insurance plans accept superbill submissions through their member portal or app. Upload the PDF, complete a brief claim form, submit. Some plans accept mail or fax submissions. Contact your insurer's member services line for their specific process.
How long does superbill reimbursement take?
Typically 2-6 weeks from submission. We don't control this timeline — your insurance does.
How much will my insurance reimburse from a superbill?
Depends on your plan's OON benefits. PPO plans often reimburse 50-70% after you meet your OON deductible. HMO and EPO plans usually reimburse nothing for OON care except in emergencies.
What if my insurance denies the superbill claim?
Common denial reasons: not meeting your OON deductible yet, missing pre-authorization (rare for outpatient psychiatry), or the plan doesn't cover OON. Denial doesn't mean you owe more — you've already paid at the time of service.
Are superbills different from regular receipts?
Yes. A regular receipt shows what you paid. A superbill includes the additional codes your insurance needs to process a reimbursement claim. We provide superbills automatically; you don't need to request them separately.
Can I use a superbill if I have Medicare or Medicaid?
Generally no. Medicare and Medicaid don't reimburse out-of-network providers via superbills the way commercial plans do. If you have Medicare or Medicaid, we'd discuss whether to bill directly or whether self-pay makes more sense for your situation.
