Why This Matters
Transferring psychiatric care is more involved than transferring most other medical care. Continuity of medication regimens, accurate diagnostic history, and an established treatment relationship all matter for outcomes. Done well, a transfer is seamless — records reach the new provider before the first visit, medication continues without interruption, and the first appointment focuses on care rather than rebuilding history from scratch.
Transfer evaluations fit patients who have established psychiatric care with another provider and want to move that care to our practice. Reasons for transfer include relocation, insurance network changes, provider retirement, dissatisfaction with the previous provider, or preference for telepsychiatry. We do not require dissatisfaction with the previous provider to accept a transfer.
