Schedule a SPRAVATO® Consultation in Tyler, TX
Your first step toward SPRAVATO® treatment is a comprehensive psychiatric evaluation at East Texas Psychiatry — same-week appointments available for most new patients.
Quick Answer
The first step to SPRAVATO® treatment at East Texas Psychiatry is a comprehensive psychiatric evaluation — not a brief intake form. Your evaluation confirms your diagnosis, reviews your full antidepressant history, assesses whether SPRAVATO® is appropriate, and initiates prior authorization with your insurance the same day. Same-week appointments are available for most new patients.
01What Is Included in a SPRAVATO® Consultation?
Your initial SPRAVATO® consultation at East Texas Psychiatry is a comprehensive psychiatric evaluation — not a quick intake screen. A board-certified psychiatric nurse practitioner conducts a structured clinical interview that covers your full psychiatric history, antidepressant treatment history, current medications, relevant medical history, and your goals for treatment.
The evaluation accomplishes several clinical functions. First, it confirms your diagnosis — or identifies nuances in your presentation that may affect treatment planning, such as a bipolar spectrum disorder, a trauma condition, or an anxiety disorder that is contributing to treatment resistance.
Second, it reviews your prior antidepressant history in the detail required for insurance prior authorization. This includes the specific medications you tried, the doses, the duration of each trial, and the reason for discontinuation. The specificity of this documentation matters for getting prior authorization approved on the first submission.
Third, it establishes your baseline symptom severity using validated rating scales. These baseline measures allow us to track your response to SPRAVATO® objectively across the induction and maintenance phases.
Fourth, it is an opportunity for you to ask questions. Many patients come to their first consultation with significant uncertainty about what SPRAVATO® is, what it feels like, what the sessions involve, and what to realistically expect. We answer those questions directly, without overselling outcomes or underplaying the commitment involved.
02How to Prepare for Your SPRAVATO® Consultation
A well-prepared patient gets more from the evaluation and produces stronger prior authorization documentation. Here is what to gather before your appointment.
Antidepressant history: A written list of every antidepressant you have tried — including approximate start and stop dates, doses (if known), prescriber name if available, and the reason each was stopped. This is the single most important piece of information for SPRAVATO® eligibility assessment and insurance prior authorization.
Current medications: A complete list of everything you currently take — prescription medications, over-the-counter medications, and supplements. Drug interactions with esketamine are relevant to your clinical assessment.
Prior psychiatric diagnoses: Any psychiatric diagnoses you have received beyond major depression — bipolar disorder, PTSD, ADHD, anxiety disorders. Bring records if available; if not, be prepared to describe the diagnosis and its context.
Insurance information: Your insurance card. For Medicare patients, bring your Medicare card and any supplemental coverage information. For Medicare Advantage, bring the Advantage plan card.
Symptom description: Think about how depression specifically affects your daily life — sleep, energy, concentration, relationships, work performance, motivation. The more specific and concrete your description, the more useful the evaluation.
Questions: Write down any questions you have — about candidacy, side effects, session logistics, cost, or anything else. You will have time to ask them during your evaluation and will get clearer answers than if you try to remember them in the moment.

03Insurance Verification and Prior Authorization — What Happens After Your Evaluation
After your evaluation, we do not ask you to navigate prior authorization yourself. Here is exactly what we do.
Same-day submission: We compile your prior authorization package the same day as your evaluation — treatment history, clinical notes, letter of medical necessity, and any insurer-specific requirements. The package goes to your insurer before the end of the business day.
Status tracking: We track the authorization status and respond immediately to any requests for additional information. Most insurers make initial decisions within 5–14 business days.
Notification: We contact you as soon as a decision is received. If it is approved, we proceed to session scheduling. If it is denied, we immediately initiate the next step — peer-to-peer discussion, additional documentation, or formal appeal.
Authorization management: We handle authorization renewals for the maintenance phase before your current authorization expires, so your sessions are never interrupted by an administrative gap.
We accept Aetna, Ambetter, Blue Cross Blue Shield, Cigna/Evernorth, Medicare, Optum, UnitedHealthcare, and UMR.
04Telepsychiatry Consultations for Out-of-Tyler Patients
If you live outside Tyler — in Longview, Nacogdoches, Lufkin, Athens, or elsewhere in East Texas or statewide — your initial SPRAVATO® consultation can happen via secure telepsychiatry.
The evaluation is conducted the same way as an in-person appointment: a comprehensive psychiatric interview, review of your treatment history, and determination of SPRAVATO® candidacy. The difference is that you participate from wherever you are in Texas — home, work, or any private location.
After a telehealth evaluation, if SPRAVATO® is determined appropriate and prior authorization is confirmed, your SPRAVATO® sessions themselves must happen at our Tyler clinic. That is a REMS requirement — esketamine can only be administered in a certified facility with monitoring capability. But your evaluation, medication management, and follow-up consultations can continue via telepsychiatry.
This means that for patients outside Tyler, the in-person commitment is limited to SPRAVATO® session days — typically two visits per week during the four-week induction phase, then weekly or biweekly during maintenance. All other care happens remotely.
The challenge
Starting a new treatment for treatment-resistant depression involves uncertainty — the consultation is your opportunity to get direct answers and make an informed decision.
Our approach
East Texas Psychiatry provides comprehensive evaluations, same-day authorization submission, and transparent discussion of what SPRAVATO® involves, what to expect, and what alternatives exist.
The outcome
Patients who understand their treatment before starting it show better engagement and outcomes than those who arrive at sessions with significant uncertainty and unresolved questions.
Take the first step
Help starts with one conversation — most new patients are seen within a week.
05From Consultation to First Session — the Timeline
Here is the realistic timeline from your first consultation to your first SPRAVATO® session.
Day 0 — Evaluation: You complete your comprehensive psychiatric evaluation. If SPRAVATO® is appropriate, prior authorization is initiated the same day.
Days 1–14 — Prior authorization: Your insurer reviews the request. Most decisions arrive within 5–14 business days. If additional documentation is needed, we respond immediately.
Day of authorization — Session scheduling: As soon as we receive authorization, we contact you to schedule your induction sessions. We work around your schedule and confirm transportation logistics.
First session: You arrive at our Tyler clinic for your first monitored SPRAVATO® session. Expect approximately two hours at the clinic.
For most patients, the time from consultation to first session is two to four weeks, depending primarily on how quickly their insurer processes the prior authorization. We do not add delays on our end.

06Questions to Ask at Your SPRAVATO® Consultation
Preparing specific questions helps you get maximum value from your consultation. Here are the questions that patients most commonly find useful to ask.
Am I a good candidate for SPRAVATO®? — The provider will assess this, but asking directly invites a direct answer and a discussion of any complicating factors.
What does a session actually feel like? — Ask your provider to describe the experience in concrete terms: what you will feel, how long different effects last, and what is considered normal versus concerning.
What is the realistic likelihood of response for someone with my history? — This opens a conversation about expectations that is more useful than a generic answer.
What is the plan if SPRAVATO® doesn't work? — Understanding the contingency plan before you start reduces anxiety about treatment failure.
How will you track whether it is working? — Understanding the monitoring process and what signals to look for helps you be a more active participant in assessing your response.
What are my total out-of-pocket costs likely to be? — Ask specifically about cost-sharing for your insurance plan and any assistance programs that may apply.
