SPRAVATO® for Depression in Tyler, TX
FDA-cleared esketamine therapy for treatment-resistant depression — REMS-certified clinic, same-week appointments, insurance accepted including Medicare.
Quick Answer
East Texas Psychiatry and Counseling offers SPRAVATO® (esketamine nasal spray) for adults with treatment-resistant depression in Tyler, TX. Treatment is administered in our REMS-certified clinic under direct supervision. Our team verifies your insurance benefits and handles prior authorization before your first session.
01What Is SPRAVATO® and Who Is It For?
SPRAVATO® (esketamine) is a nasal spray derived from ketamine, FDA-approved in 2019 for treatment-resistant depression (TRD) and in 2020 for major depressive disorder with active suicidal ideation (MDSI). It represents a significant clinical advance for people whose depression has not responded to standard antidepressants.
Traditional antidepressants — SSRIs, SNRIs, tricyclics — work by increasing serotonin, norepinephrine, or dopamine availability. SPRAVATO® works differently: it blocks NMDA receptors, triggering rapid synaptogenesis — growth of new synaptic connections — in brain regions associated with mood regulation. This mechanism produces antidepressant effects faster than standard medications and helps patients for whom monoamine-based treatments have not worked.
At East Texas Psychiatry, SPRAVATO® is integrated into a comprehensive psychiatric care plan that typically includes medication management and therapy coordination. It is not a standalone fix — it is one tool in a well-built treatment strategy.
Candidacy requires documented failure of at least two antidepressants at adequate doses for adequate durations. Our comprehensive evaluation confirms eligibility, maps your treatment history, and builds a plan specific to your situation and goals.
02Signs and Symptoms of Treatment-Resistant Depression
Treatment-resistant depression (TRD) is a clinical designation, not a vague description. The standard definition requires major depression that has not responded to at least two antidepressant medications taken at adequate doses for adequate durations — typically eight to twelve weeks each.
People living with TRD often experience persistent low mood lasting months or years despite medication trials, significant fatigue and loss of motivation, cognitive difficulties including poor concentration and memory gaps, sleep disruption (often excessive sleeping rather than insomnia), social withdrawal, disengagement from previously meaningful activities, and in severe cases, passive suicidal ideation.
TRD affects roughly 30% of people diagnosed with major depressive disorder. For many years the options were limited: try another antidepressant, add an augmenting agent, or consider electroconvulsive therapy. SPRAVATO® provides a genuinely different mechanism that can work when others have not.
Distinguishing TRD from depression complicated by an undiagnosed condition — bipolar disorder, ADHD, a trauma-spectrum condition — is also part of what a thorough evaluation accomplishes. Treating the wrong diagnosis explains many cases of apparent treatment resistance. Our evaluation is designed to catch these complexities before initiating any new treatment.
| Question | What to know |
|---|---|
| FDA-approved for TRD? | Yes — 2019 for TRD; 2020 for MDD with acute suicidal ideation. |
| How administered? | Nasal spray, self-administered under staff supervision in our Tyler clinic. |
| Session length? | ~2 hours including mandatory post-dose monitoring. |
| Frequency? | 2× weekly × 4 weeks (induction); then weekly or biweekly (maintenance). |
| Does insurance cover it? | Medicare Part B and most major commercial plans with prior authorization. |
| Same-week start? | Consultations typically available within a week of first contact. |
03Causes and Risk Factors for Treatment Resistance
Treatment resistance reflects the biological complexity of depression, not a patient's failure to respond correctly. Several factors are associated with higher rates of non-response to standard antidepressants.
Pharmacogenomic variation — genetic differences in how individuals metabolize psychiatric medications — means that standard doses of a given antidepressant may be ineffective or poorly tolerated in certain patients. We offer pharmacogenomic testing when this may explain a pattern of treatment failure.
Undiagnosed comorbidities frequently complicate depression treatment. Bipolar spectrum disorder, anxiety disorders, ADHD, PTSD, and chronic pain all share symptoms with unipolar depression. Treating depression without addressing co-occurring conditions often produces incomplete or unstable responses.
Inadequate prior treatment — doses at the lower end of the therapeutic range, trials that were stopped too soon, or combinations that were not optimized — can create an apparent pattern of non-response that is actually addressable without SPRAVATO®. Our evaluation distinguishes between genuine TRD and undertreated depression.
Psychosocial factors including chronic stress, unresolved trauma, and relationship difficulty can maintain depressive symptoms even when medication is partially effective. SPRAVATO® is most effective as part of a broader treatment plan that addresses these dimensions alongside the biological aspects of depression.
04How East Texas Psychiatry Delivers SPRAVATO® Care
Our SPRAVATO® program begins with a comprehensive psychiatric evaluation that covers your full medication history, prior antidepressant trials, current symptoms, health background, and goals. This evaluation confirms eligibility, identifies co-occurring conditions, and produces a care plan specific to your situation.
After the evaluation, we initiate prior authorization with your insurer the same day — submitting complete documentation including your treatment history and a letter of medical necessity. Most authorizations for patients with commercial insurance or Medicare Part B are approved; we manage the full process including peer-to-peer discussions and appeals when needed.
The SPRAVATO® induction phase is twice-weekly sessions for four weeks — eight sessions total. Each session runs approximately two hours, including the mandatory REMS monitoring period. We schedule sessions around your work and family obligations as much as possible.
After induction, patients who respond move to maintenance sessions — typically weekly or biweekly — with frequency adjusted based on your response. We use validated depression rating scales across treatment to track symptom change objectively and make evidence-based adjustments.
We accept Aetna, Ambetter, Blue Cross Blue Shield, Cigna/Evernorth, Medicare, Optum, UnitedHealthcare, and UMR. For uninsured or underinsured patients, we discuss the Janssen CarePath savings program at your initial appointment.
The challenge
Left untreated, treatment-resistant depression compounds over time — eroding relationships, employment, and physical health in ways that become harder to reverse.
Our approach
East Texas Psychiatry provides a complete evaluation, manages prior authorization, delivers monitored SPRAVATO® sessions, and coordinates ongoing care — all under one roof.
The outcome
SPRAVATO® clinical trials showed meaningful response rates in TRD patients within the four-week induction phase — in a population that had already failed multiple antidepressant treatments.

05What Sets Our SPRAVATO® Program Apart
East Texas Psychiatry and Counseling is one of a limited number of REMS-certified practices in East Texas. This certification — required by the FDA — means our facility, staff, and processes have met federal standards for safe esketamine administration.
Our clinical team brings substantial depth: James Baughman, MSN APRN PMHNP-BC, co-founded the practice with more than 35 years in psychiatric nursing and U.S. Navy service. Antonio Brigham, MSN-Ed APRN PMHNP-BC PMHNP-C, holds dual board certification. Karen A. English, MSN PMHNP-BC, specializes in child and adolescent psychiatry.
Same-week appointments are a practical commitment — not a marketing claim. We structure scheduling specifically to accommodate new patients promptly. Waiting months to start a new treatment for a serious condition is not acceptable to us.
For patients outside Tyler, we offer statewide telepsychiatry for evaluations, medication management, and follow-up care. The SPRAVATO® sessions themselves require in-person visits to our Tyler clinic — that is a REMS requirement — but all other components of your care can be managed remotely.

06What to Expect at Your SPRAVATO® Sessions
Each SPRAVATO® session at East Texas Psychiatry follows a consistent, REMS-compliant protocol. Knowing what to expect makes starting treatment less uncertain.
Before each session: You confirm with staff that your health status is unchanged and you are feeling well enough to receive a dose. Plan not to drive for the rest of the day — arrange transportation for every session day.
Medication administration: The nasal spray is self-administered with staff present in the room. Doses are divided and delivered across the session. You are never alone during administration.
Two-hour monitoring period: You rest in a private, comfortable space. You may experience dissociation, dizziness, or nausea — these are expected, transient, and monitored. They resolve within the monitoring period for most patients. Staff check on you regularly and are immediately available if needed.
Discharge: We confirm you are stable before you leave. You will not drive yourself home.
Most patients describe sessions as manageable, particularly after the first one or two, when the experience becomes more familiar. The therapeutic work of integrating changes happens between sessions — in your daily life and in any therapy you are engaged in.
