SPRAVATO® for Mental Health in Tyler, TX
East Texas Psychiatry integrates SPRAVATO® into comprehensive mental health care — not as a standalone procedure, but as part of a coordinated plan that addresses the full picture.
Quick Answer
At East Texas Psychiatry, SPRAVATO® is administered within a comprehensive psychiatric care framework in Tyler, TX. Esketamine is most effective when integrated with medication management, therapy coordination, and ongoing psychiatric support. Our team builds a care plan that addresses your full mental health history, not just the SPRAVATO® sessions.
01SPRAVATO® in Context — Part of a Care Plan, Not a Standalone Fix
SPRAVATO® is a powerful clinical tool, but it is most effective when used as part of a well-constructed psychiatric care plan — not as a standalone procedure administered in isolation from the rest of your mental health care.
The clinical trials that established SPRAVATO®'s efficacy tested it in combination with an oral antidepressant. In clinical practice, the best outcomes are typically seen when esketamine is coordinated with ongoing medication management, and where possible, with psychotherapy. The synaptogenesis that SPRAVATO® triggers creates a window of enhanced neuroplasticity — a period during which the brain may be more responsive to other therapeutic inputs, including therapy and behavioral change.
This integrated approach is not just a theoretical preference — it is how mental health conditions actually work. Depression with co-occurring anxiety, PTSD, ADHD, or chronic pain does not get fully better when only one component is addressed. SPRAVATO® may lift the floor of depression quickly and meaningfully, but the work of building a stable, sustainable mental health foundation requires the full architecture of care.
At East Texas Psychiatry, we do not run SPRAVATO® sessions as a separate, isolated service. We integrate them into an ongoing psychiatric care relationship that includes regular check-ins, medication management, symptom tracking with validated measures, and coordination with therapy when relevant.
02Conditions We Treat Alongside SPRAVATO®
Most patients who receive SPRAVATO® at East Texas Psychiatry have more than one active mental health condition. Treatment-resistant depression rarely exists in isolation — it is most commonly accompanied by anxiety disorders, PTSD, ADHD, chronic pain, or other conditions that both complicate depression treatment and require their own clinical attention.
Major depressive disorder and TRD: The primary indication for SPRAVATO®. We evaluate the full presentation — not just whether you have tried two antidepressants, but what your depression has looked like over time, what has helped partially, and what other factors are maintaining it.
Anxiety disorders: Generalized anxiety disorder, panic disorder, social anxiety, and PTSD all frequently co-occur with major depression. Anxiety that is undertreated can maintain depression even when antidepressant medication is otherwise working. We treat anxiety alongside depression in a coordinated way.
PTSD: Post-traumatic stress disorder and depression form a particularly common and treatment-complicating combination. We assess for PTSD at your initial evaluation and incorporate trauma-related factors into your care plan.
ADHD: Adult ADHD is frequently underdiagnosed and, when untreated, can maintain cognitive symptoms that look like depression and undermine antidepressant response. We assess for ADHD and treat it when present.
Mood disorders: Bipolar spectrum disorders require careful distinction from major depression because the treatment approach differs significantly. We evaluate for bipolar spectrum presentations at your initial evaluation.

03Integrating SPRAVATO® With Psychotherapy
Psychotherapy — particularly cognitive behavioral therapy (CBT), acceptance and commitment therapy (ACT), and trauma-focused modalities like EMDR — is an evidence-based treatment for depression that works through different mechanisms than pharmacotherapy. When combined with SPRAVATO®, there is reason to believe the two may be synergistic.
The neuroplasticity window following SPRAVATO® sessions — the period of enhanced synaptic growth and brain plasticity — may be a particularly productive time for therapeutic work. Some researchers and clinicians describe this as a window of enhanced "openness" to new learning and perspective, which could support the cognitive and behavioral changes that therapy aims to produce.
Practically, this means that patients who are engaged in therapy alongside SPRAVATO® treatment should consider scheduling therapy appointments in the days following sessions rather than immediately before, to potentially take advantage of the neuroplastic state.
East Texas Psychiatry does not provide psychotherapy directly, but we coordinate with therapists and can provide referrals to therapists experienced in treating depression and trauma in Tyler and across East Texas. If you already have a therapist, we coordinate with them with your consent to ensure your care is coherent across providers.
04Medication Management Alongside SPRAVATO®
SPRAVATO® is almost always prescribed alongside an oral antidepressant — this is the clinically validated approach, consistent with how the pivotal trials were designed. Managing the full medication regimen across SPRAVATO® induction and maintenance requires ongoing clinical attention.
At your initial evaluation, we review your complete medication list for interaction potential and therapeutic optimization. If your current antidepressant regimen is not optimized, we may recommend adjustments before or alongside starting SPRAVATO®.
During induction, we monitor how you are tolerating your full regimen — not just the esketamine sessions but the oral antidepressants and any other psychiatric medications. We use validated symptom measures at key points to track what is changing and make evidence-based decisions.
As you move into maintenance and your clinical status evolves, your medication plan should evolve with it. Patients who achieve robust remission with SPRAVATO® may be able to simplify their oral antidepressant regimen over time. Patients who have a more partial response may need augmentation strategies. We manage these decisions collaboratively and explicitly.
We accept Aetna, Ambetter, Blue Cross Blue Shield, Cigna/Evernorth, Medicare, Optum, UnitedHealthcare, and UMR.

06Taking a Whole-Person Approach
Mental health conditions do not exist independently of the rest of a person's life — and effective psychiatric care does not treat them as if they do. At East Texas Psychiatry, we take a whole-person approach to SPRAVATO® care and to psychiatric care generally.
This means that your evaluation includes not just your symptom history and medication list, but also your life context — your relationships, your work situation, your sleep, your physical health, your history. These factors shape both the presentation of your condition and the treatment plan most likely to help.
It also means that we coordinate with your other healthcare providers when relevant — primary care physicians, specialists, therapists — to ensure that your psychiatric care is coherent with your broader health picture.
And it means that we talk honestly with you about what SPRAVATO® can and cannot do, what realistic outcomes look like for someone with your specific history, and what the path forward looks like beyond the initial treatment course.
