REMS-Certified SPRAVATO® Care in Tyler, TX
East Texas Psychiatry holds federal REMS certification — the FDA safety requirement for administering SPRAVATO® (esketamine) in Tyler, TX.
Quick Answer
REMS (Risk Evaluation and Mitigation Strategy) certification is a federal requirement for any practice that administers SPRAVATO®. East Texas Psychiatry and Counseling has completed the facility, training, and monitoring requirements for REMS certification. You cannot legally receive SPRAVATO® at a non-REMS-certified practice.
01What Is the SPRAVATO® REMS Program?
The SPRAVATO® REMS (Risk Evaluation and Mitigation Strategy) program is a federal safety framework established by the FDA as a condition of esketamine's approval. REMS programs exist for medications that provide meaningful clinical benefit but carry specific risks requiring structured safeguards. For SPRAVATO®, those risks center on dissociation, sedation, and misuse potential.
The REMS program applies to the manufacturer (Janssen Pharmaceuticals), to certified healthcare settings, and to the patients who receive the drug. All three must be enrolled and compliant for a session to legally take place.
For healthcare settings, REMS certification requires completion of a certified training program covering esketamine pharmacology and monitoring protocols, a facility equipped to observe patients for at least two hours post-dose with capability to manage adverse outcomes, use of the SPRAVATO® Risk Acknowledgment Form with every patient, and enrollment in the REMS registry with ongoing compliance reporting.
REMS certification is not a one-time credential — it requires ongoing compliance. Practices that do not maintain the standards lose authorization to administer SPRAVATO®. Choosing a REMS-certified practice means choosing a setting held to and continuing to meet federal safety standards for esketamine administration.
02Why REMS Certification Matters for Patients
For patients considering SPRAVATO®, REMS certification is not bureaucratic detail — it is the mechanism that makes esketamine safe to deliver in an outpatient setting.
Esketamine has dissociative properties. During and after administration, patients may experience altered perception, dissociation (a sense of detachment from surroundings), dizziness, or sedation. These effects are transient and clinically manageable in a REMS-certified setting with trained staff. They are not safe in an unmonitored environment — which is why home administration is prohibited under REMS.
REMS also addresses misuse risk. Ketamine-derived substances have a history of recreational misuse. REMS controls on how SPRAVATO® is dispensed and administered prevent diversion — you will not receive a prescription to fill at a pharmacy. Each dose is administered in the clinic and the medication remains with the practice.
From an insurance standpoint, Medicare and most commercial payers require SPRAVATO® to be administered in a REMS-certified setting. Receiving esketamine at a non-certified facility would be a compliance violation affecting both the practice and your coverage.
REMS certification also signals to patients and referring providers that a practice has made an affirmative commitment to the standards of safe esketamine care — not just that they have access to the drug.
| Question | What to know |
|---|---|
| Facility requirements | Monitoring capability, adverse event management, private patient space. |
| Staff training | Certified REMS training on pharmacology, risks, and monitoring protocols. |
| Patient enrollment | Every patient enrolled in REMS registry before first dose. |
| Informed consent | Risk acknowledgment form and substantive consent discussion required. |
| Post-dose monitoring | Minimum 2 hours after every dose — federal requirement, no exceptions. |
| No home use | Each session must occur at the certified facility; home administration prohibited. |
03What the REMS Monitoring Requirement Means in Practice
The most operationally significant REMS requirement is the two-hour post-dose monitoring period. This is not a guideline or a recommendation — it is a federal requirement for every SPRAVATO® session without exception.
During this monitoring period, patients rest in the clinical setting while staff observe for and manage dissociation, sedation, dizziness, nausea, and blood pressure changes. These effects are expected and predictable — their transient nature is well-established in clinical trials. The monitoring period is specifically designed to cover the window during which these effects are most prominent.
For patients, this means planning approximately two hours at the clinic for every session, plus travel time. During induction (twice weekly for four weeks), that is roughly four hours per week in our Tyler facility. After induction, sessions shift to weekly or biweekly maintenance, which reduces the time commitment.
The monitoring period cannot be shortened. Patients who feel well before the two hours are complete still must complete the full period before discharge. This is both a REMS requirement and sound clinical practice — the dissociative effects of esketamine can linger in less obvious forms even when the patient subjectively feels recovered.
Transportation is also a REMS-related safety requirement. You cannot drive after any SPRAVATO® session. Arrange transportation for every session day — this is non-negotiable.
04East Texas Psychiatry's REMS-Compliant Process
Every aspect of SPRAVATO® care at East Texas Psychiatry is REMS-compliant. Here is how certification translates into your experience as a patient.
Enrollment: Before your first session, you complete the SPRAVATO® REMS enrollment process, which includes a substantive informed consent discussion about the risks, benefits, and requirements of esketamine treatment. This is a real conversation with your provider, not a form to sign.
Facility: Our Tyler clinic provides private monitoring space with trained staff available throughout the two-hour period. The environment is designed for comfort — we recognize that rest and a calm setting support the therapeutic process, not just the safety requirement.
Session protocol: Each session follows a REMS-compliant sequence — pre-session assessment, medication administration, monitored rest, post-monitoring assessment, and discharge. We do not cut the monitoring period short for any reason.
Registry reporting: After each session, we document your response and any adverse effects in the REMS registry as required. This supports ongoing pharmacovigilance and the FDA's ability to monitor real-world safety data.
We accept Aetna, Ambetter, Blue Cross Blue Shield, Cigna/Evernorth, Medicare, Optum, UnitedHealthcare, and UMR.
The challenge
Choosing a non-REMS-certified setting is not just a regulatory issue — it is a patient safety issue. Without proper monitoring, the acute effects of esketamine cannot be safely managed.
Our approach
East Texas Psychiatry is REMS-certified: facility, staff, and protocols have met federal safety standards. You receive SPRAVATO® in a safe, monitored, compliant environment.
The outcome
Patients treated in REMS-compliant settings have a well-established safety profile that supports outpatient administration of a treatment that previously required inpatient settings.

05Our REMS Clinical Team
REMS certification covers our facility and processes — but the clinical team behind them is what makes SPRAVATO® care genuinely good rather than just compliant.
James Baughman, MSN APRN PMHNP-BC, co-founded East Texas Psychiatry with more than 35 years of psychiatric nursing experience, including U.S. Navy service. His depth of experience with complex and treatment-resistant conditions makes him well-suited to the patient population who most benefits from SPRAVATO®.
Antonio Brigham, MSN-Ed APRN PMHNP-BC PMHNP-C, holds dual board certification and brings expertise in holistic psychiatric approaches. Karen A. English, MSN PMHNP-BC, specializes in child and adolescent psychiatry.
Together, this team provides SPRAVATO® care within a comprehensive psychiatric practice — not a standalone infusion clinic. Your SPRAVATO® treatment is coordinated with your full psychiatric care plan, including medication management and therapy referrals where appropriate.
Same-week consultation appointments are available for most new patients. Statewide telepsychiatry is available for components of care that do not require an in-person visit.

06Planning for REMS-Compliant Sessions
Starting SPRAVATO® treatment involves a logistics commitment that is worth planning for clearly before you begin.
Transportation: Every session day requires arranged transportation — to and from the clinic. You cannot drive after any session. Many patients arrange a standing ride with a family member or friend during the induction phase.
Schedule: Two-hour sessions twice weekly for four weeks is the induction commitment. We work to schedule sessions at times that minimize disruption to your work or family obligations — early mornings or late afternoons when possible.
Eating: A light meal two hours before a session typically helps reduce nausea. Avoid a large meal immediately before your session.
What to bring: Comfortable clothing, earphones for music or podcasts, and any comfort items that help you rest. Some patients find a specific playlist or guided meditation helpful during the monitoring period.
Expectations: The first session is the most uncertain. Most patients report that subsequent sessions feel more familiar and manageable. The dissociative effects, if experienced, typically become less intense over the course of the induction phase for many patients.
