Quick Answer

East Texas Psychiatry and Counseling serves veterans with treatment-resistant depression in Tyler, TX. Our co-founder James Baughman is a U.S. Navy veteran with 35+ years in psychiatric nursing. SPRAVATO® is FDA-approved for TRD and MDD with active suicidal ideation — conditions disproportionately prevalent among veterans. We accept Medicare, which covers SPRAVATO® under Part B.

01Depression and PTSD in Veterans — A Complex Clinical Picture

Veterans are disproportionately affected by treatment-resistant depression, PTSD, and the co-occurrence of both. The mechanisms are multiple: combat exposure and moral injury, the difficulty of transitioning from military to civilian identity, chronic pain and traumatic brain injury, and the specific ways that military culture shapes how distress is perceived and disclosed.

Depression in veterans is often complicated. PTSD frequently co-occurs with major depression, and the two conditions interact in ways that can complicate both diagnosis and treatment. Antidepressants that work reasonably well for pure major depression may have more limited efficacy in the context of PTSD co-occurrence.

Sleep disruption — common in both depression and PTSD — can perpetuate both conditions. Chronic pain is highly prevalent among veterans who have served in combat roles and can maintain depressive symptoms even when depression-specific treatment is otherwise adequate. Substance use disorders co-occur at higher rates in veterans than in the general population and must be addressed as part of treatment planning.

Veterans who have tried multiple antidepressants without adequate relief are the core population for whom SPRAVATO® was developed. The TRD designation — inadequate response to two or more antidepressants at adequate doses and durations — applies to a significant proportion of veterans with depression who present for specialty psychiatric care.

Treatment-Resistant Depression in Veterans — Studies suggest that rates of treatment-resistant depression are higher among veterans than in the general population, driven by higher rates of PTSD co-occurrence, traumatic brain injury, chronic pain, and related comorbidities that complicate standard depression treatment.

02How SPRAVATO® May Help Veterans With TRD

SPRAVATO®'s mechanism — NMDA receptor modulation triggering rapid synaptogenesis — may be particularly relevant for veterans with treatment-resistant depression for several reasons.

Speed of action: Traditional antidepressants require weeks to reach therapeutic effect. For veterans in acute distress or with active suicidal ideation (the MDSI indication), faster pharmacological intervention matters. SPRAVATO® can produce meaningful antidepressant effects within days of the first session for some patients.

PTSD-related depression: There is emerging evidence that ketamine-derived compounds may affect the neural circuitry involved in trauma memory consolidation and extinction — pathways that are central to PTSD. While SPRAVATO® is not FDA-approved for PTSD specifically, veterans whose depression is intertwined with PTSD symptomatology may experience benefits that extend beyond depression relief alone.

Suicidal ideation: Veterans face elevated rates of suicidal ideation and death by suicide compared to the general population. SPRAVATO® is FDA-approved for MDD with active suicidal ideation — one of the two approved indications — making it a potentially important tool for veterans in acute psychiatric crisis who have not responded to standard treatments.

Chronic pain interaction: Some research suggests that NMDA receptor involvement in chronic pain processing may be relevant to the partial analgesic effects observed with ketamine compounds. This is not an established indication for SPRAVATO® but may be relevant to veteran patients for whom chronic pain is a significant contributor to their clinical picture.

03Insurance Coverage for Veterans — Medicare and Beyond

Most veterans who receive care at East Texas Psychiatry use Medicare as their primary or secondary insurance. Medicare Part B covers SPRAVATO® as a physician-administered drug, with prior authorization required. We manage the full prior authorization process — documentation, submission, peer-to-peer, and appeals.

For veterans enrolled in VA healthcare: SPRAVATO® access through the VA system has been expanding but can vary significantly by facility and region. Some veterans find it faster and more straightforward to access SPRAVATO® through a private community provider like East Texas Psychiatry, using Medicare as their payer, rather than navigating VA authorization.

For veterans without Medicare who have commercial insurance: We accept Aetna, Ambetter, Blue Cross Blue Shield, Cigna/Evernorth, Medicare, Optum, UnitedHealthcare, and UMR. If you have employer-sponsored insurance or insurance through a spouse, we verify your specific coverage and manage prior authorization accordingly.

For uninsured veterans: Janssen CarePath offers cost assistance for eligible patients. We discuss this program at your initial appointment.

Community Care Network (CCN): Veterans who are VA-enrolled and live more than a certain distance from a VA facility may be eligible to receive care from community providers like East Texas Psychiatry with VA authorization. Call us to discuss whether this pathway applies to your situation.

04Our Experience With Veterans and Military Culture

Our co-founder James Baughman, MSN APRN PMHNP-BC, served in the U.S. Navy. This experience informs how our practice understands military culture — not as an outsider's interpretation, but as someone who has lived it.

Veterans sometimes encounter civilian healthcare providers who are unfamiliar with military culture and who inadvertently use language or frameworks that feel disconnected from service experience. We do not approach veterans as a generic patient population — we understand the culture of service, the difficulty of asking for help in a culture that values self-reliance, and the specific ways that military experience shapes mental health presentation and treatment engagement.

We also understand that stigma around mental health remains a barrier for some veterans, and that a conversation about depression or PTSD may be something a veteran has been putting off for a long time before they call us. We meet that moment with directness and respect — not with clinical platitudes.

Same-week appointments are available. Call (430) 288-5800 or use the Schedule button. Statewide telepsychiatry is available for veterans outside Tyler who prefer a remote initial evaluation.

05SPRAVATO® at East Texas Psychiatry — What Veterans Can Expect

The SPRAVATO® process at East Texas Psychiatry is the same for veterans as for any other patient — a comprehensive evaluation, prior authorization management, and REMS-compliant sessions in our Tyler clinic. What differs is that our team brings a specific understanding of veterans' clinical presentations and the factors that shape their treatment histories.

At your evaluation, we take a thorough psychiatric history that includes your service history and deployment experience, not just your symptom timeline. We assess for PTSD co-occurrence, chronic pain, sleep disruption, and other veteran-specific comorbidities that affect depression treatment.

We discuss realistic expectations: SPRAVATO® has meaningful efficacy data in TRD populations, but it is not guaranteed to work for every patient, and it works best as part of a comprehensive treatment plan that addresses the full range of factors contributing to treatment resistance.

We also coordinate with VA providers when relevant. If you have a VA psychiatrist or primary care team, we can share clinical information with their consent and work as a community provider within your broader care network.
